Submitting an Absence Note

Documentation following an Absence (Full or Partial day absence)

Within three (3) school days of the student’s arrival or return to school, a note signed by the parent that describes the reason for the absence must be delivered to the appropriate campus office for attendance documentation. Documentation must support the reason for the absence to be coded correctly.

Please Note: A note signed by the student or submitted via a student email address, even with the parent’s permission, will not be accepted unless the student is 18 or older or is an emancipated minor under state law. If the absence is due to a communicable disease excludable by the DSHS, a physician’s note will be required. The campus will document in its attendance records for the student whether the absence is considered excused or unexcused.

Should the student develop a questionable pattern of absences, the principal or attendance committee may require a statement from a doctor or health clinic verifying the illness or condition that caused the student’s absence from school and prohibit parent notes for future absences.

Note: The district is not required to excuse any absence, even if the parent provides a note explaining the absence, unless the absence is an exemption under compulsory attendance laws.

Doctor’s Note after an Absence for Illness (All Grade Levels)

Upon returning to school, a student absent for more than 5 consecutive days because of a personal illness must bring a statement from a doctor or health clinic verifying the illness or condition that caused the absence. Otherwise, the absence may be considered unexcused and in violation of compulsory attendance laws.

Should the student develop a questionable pattern of absences, the principal or attendance committee may require a statement from a doctor or health clinic verifying the illness or condition that caused the absence to determine whether an absence will be excused or unexcused.
